Mileage Reimbursement Form

Because Heartland Christian School is accredited by the State of Iowa, you may be eligible to receive reimbursement money for transporting your own child to school. Please fill out the forms below and return them to your local school district. A list of school districts and their addresses may also be found in the link. You must submit the form twice a school year calendar.

Deadlines for submitting reimbursement forms:
December 1 (first semester reimbursement)
May 1 (second semester reimbursement)

Note: In the blank for "miles to school" put 2 miles or more. DO NOT put less than 2 miles.
